Disclosure Document Assignment

Step 1: Read Disclosure document with parents
Step 2: Parents and Students sign the Disclosure Signature Page and initial the           
​                    Calendar Assignment, then turn them in to Mrs. Thomson by Friday, 
​                     August 22nd
Step 3: If you are in an advanced choir, sign the Advanced Choir Commitment sheet and turn it in to Mrs. Thomson
